Many patients have health insurance that provides reimbursement for some or most of the cost of physical therapy. To determine the extent of your insurance coverage, you should review your insurance contract or call the customer service number on the back of your insurance card to determine if physical therapy services outside of network are covered. Although I do not accept most insurance, I can provide you with a receipt to submit to your insurance company.
All of my services can be paid for using an FSA or HSA. I would be happy to provide you with a receipt.
No! Minnesota is a “direct access” state meaning that you are able to directly see a physical therapist for 90 days prior to needing a prescription from your physician.
After you have scheduled your initial evaluation, you will receive an e-mail to complete necessary online documents (health history and consent for treatment). Please complete these prior to arriving for your visit.
If you have one, please bring your physical therapy prescription from your physician and/or any other pertinent diagnostic reports (X-ray, MRI).
Wear comfortable and loose-fitting clothing that allows you to move.
Your therapy plan of care is completely customized to you. Based on my assessment and your goals we will come up with the perfect plan at your initial visit.
In order to minimize the risk of Covid-19 transmission, I am following CDC guidelines. I ask that you wear a mask throughout your treatment. If you forget a mask, one will be provided for you. If you have any cold or flu-like symptoms, please stay home.
