Agreement to These Terms
These Terms & Conditions ("Terms") govern your use of meleahmurphypt.com (the "Site"), operated by Meleah Murphy Physical Therapy ("we," "us," or "our"). By accessing or using the Site, you agree to these Terms. If you do not agree, please do not use the Site.
Not Medical Advice
The Site provides general educational information about physical therapy and active living. It is not medical advice, a diagnosis, or a treatment plan, and using the Site does not create a provider-patient relationship. Always consult a qualified healthcare provider about your individual condition, and never disregard or delay seeking professional care because of something you read here.
Care and Outcomes
Physical therapy outcomes vary from person to person and depend on many individual factors. Information on the Site reflects general clinical knowledge and experience, but it cannot predict your personal results, and no specific outcome is promised or guaranteed.
Appointments and Scheduling
Appointments are booked through our third-party scheduling provider, Jane (Jane App), hosted at the Guided Health booking page. Submitting a request or selecting a time is subject to confirmation and to the policies of the clinic, including any cancellation, rescheduling, or no-show policies provided to you. We are not responsible for the availability or operation of the Jane platform. Any contact form submission on this Site is a request to get in touch, not a confirmed appointment.
Payment and Insurance
Meleah Murphy Physical Therapy is a cash-based practice and does not bill insurance directly. Payment is due at the time of service. Services are generally eligible for payment through HSA or FSA accounts, and a superbill can be provided on request for you to submit to your insurer for possible out-of-network reimbursement. We do not guarantee that any insurer will reimburse any amount. Minnesota is a direct-access state, so a physician referral is generally not required to be seen; however, you are responsible for confirming any referral or authorization your own insurance plan may require for reimbursement.
Intellectual Property
Unless otherwise noted, all content on the Site, including text, images, graphics, logos, and the overall design, is owned by Meleah Murphy Physical Therapy and protected by copyright and other laws. You may view and share content for personal, non-commercial use, with appropriate credit and a link back to the original page. You may not republish, sell, or use our content commercially, or present it as your own, without our prior written permission.
Third-Party Links and Services
The Site may link to or embed content from third parties (for example, scheduling, social media, or other tools). We provide these for convenience and do not control them. We are not responsible for the content, accuracy, privacy practices, or policies of any third-party site or service, and your use of them is at your own risk and subject to their terms.
Acceptable Use
You agree not to use the Site to:
violate any law or regulation;
disrupt, overload, or interfere with the Site or its security;

Governing Law
These Terms are governed by the laws of the State of Minnesota, without regard to its conflict-of-laws rules. Any dispute relating to these Terms or the Site will be brought exclusively in the state or federal courts located in Hennepin County, Minnesota, and you consent to their jurisdiction.
